The week's two strongest results are a pivotal oral angioedema trial and a Parkinson's trial in which a supplement did worse than placebo
The week
RAPIDe-3 leads on a scheduled April decision, and the NOPARK trial of nicotinamide riboside ranks second because a worse-than-placebo result for a widely sold supplement is as relevant to its listed maker as a positive one would have been. Below them the week is single-arm oncology, an inferred device read from the largest randomised tomosynthesis screening trial, and three results that were topline months ago. Four of the ten were first disclosures; the rest were public before their papers, three of them by more than a year.

What the paper showed
In RAPIDe-3, 134 participants with hereditary angioedema were randomised to treat two attacks in a crossover sequence; 113 treated 201 attacks and 88 had paired attacks for the primary analysis. Median time to onset of symptom relief was 1.28 hours (95% CI, 1.05 to 1.52) with oral deucrictibant 20 mg against more than 12 hours (5.82 to more than 12) with placebo (P<0.0001), no treatment-related adverse event was severe or serious, and none led to discontinuation.
Why it scores where it does
This is the pivotal trial behind an application the FDA is reviewing with an action date of 23 April 2027, for what would be the first oral bradykinin B2 receptor antagonist for on-demand treatment of attacks, in a market served by injected and infused products from larger companies. The effect is large, the crossover design sets each participant against themselves, and the only adverse event reported more than once within three days was fatigue in two participants. The topline was released on 3 December 2025, so the paper adds the full dataset and a Lancet comment rather than new direction, the action date is six months out, and the sponsor is a single-asset developer whose value is almost entirely this molecule in its two formulations.
What would resolve it
FDA action date of 23 April 2027 for the immediate-release formulation. The European application was validated in July 2026 with no decision date published.
Among 393 patients with early Parkinson disease analysed after 52 weeks, the total MDS-UPDRS score rose 2.45 points (95% CI, 0.80 to 4.10) with nicotinamide riboside 500 mg twice daily and fell 0.27 points (95% CI, -1.95 to 1.40) with placebo, an adjusted difference of 2.72 points (95% CI, 0.47 to 4.98; P=0.02) favouring placebo. Non-motor symptom burden also worsened more on treatment (adjusted difference 4.22 points; 95% CI, 1.07 to 7.37; P=0.009), and serious adverse events occurred in 8.3% against 14.1%.
Why it scores where it does
A randomised phase 3 in which the supplement did measurably worse than placebo on the primary outcome ends the disease-modification case for the NAD precursor in Parkinson disease and bears on the reputation of a product whose listed maker is a pure play on it. The trial was academic, run at 11 Norwegian centres and publicly funded; the company enters because it supplied the nicotinamide riboside and the placebo, which is a documented role rather than an inference from product overlap. The difference of 2.72 points is below the 5.2-point threshold the trial defined as clinically important worsening, the company's revenue comes from wellness use rather than any Parkinson claim, and the paper appears to be the first disclosure, so what moves is the narrative around the molecule rather than a label.
What would resolve it
No scheduled resolving event. Company commentary on its Parkinson disease programme and any further trials of the compound.

What the paper showed
Among 28 patients with transurethrally unresectable ERBB2-positive very-high-risk non-muscle-invasive bladder cancer who were ineligible for or declined cystectomy, complete response of high-risk disease occurred in 19 of 26 efficacy-evaluable patients (73.1%; 95% CI, 54.8 to 91.3), meeting the prespecified 70% goal, with a 24-month sustained response of 83.5% among responders and no muscle-invasive or metastatic progression at a median follow-up of 29.4 months. Grade 3 to 4 treatment-related adverse events occurred in 4 patients (14.3%) and 2 (7.1%) discontinued.
Why it scores where it does
A bladder-sparing systemic regimen in a population whose standard is removal of the bladder would widen the use of an antibody-drug conjugate that is the lead asset of its Chinese owner and a breakthrough-designated asset in urothelial cancer for the partner that holds rights outside Asia. Durable complete responses with modest toxicity support the case for pairing an ERBB2 conjugate with checkpoint blockade. The trial is single-arm at one academic hospital with 28 patients, the owner is not the sponsor, the checkpoint antibody belongs to a second company for which this is a minor line, and the authors call for randomised confirmation; no earlier presentation was found, so the paper is treated as the first disclosure.
What would resolve it

What the paper showed
Among 98,755 women who screened negative in the first round, invasive interval cancers within 24 months occurred in 70 of 49,299 in the tomosynthesis plus synthesised mammography group (14.2 per 10,000) and 92 of 49,456 in the digital mammography group (18.6 per 10,000; adjusted odds ratio 0.76; 95% CI, 0.55 to 1.06; P=0.11). Within 12 months the rates were 4.5 against 6.3 per 10,000 (odds ratio 0.71; 95% CI, 0.52 to 0.97).
Why it scores where it does
Whether tomosynthesis prevents the cancers that surface between screens is the question gating conversion of national screening programmes from two-dimensional mammography, and this is the largest randomised test of it, with an accompanying editorial asking whether digital mammography should be retired. About a quarter fewer interval cancers over two years and a significant reduction within twelve months favour the makers of tomosynthesis systems. The primary 24-month endpoint missed statistical significance, the trial is academic and names no vendor, and the listed companies are mapped by their share of the tomosynthesis equipment market rather than by any involvement, so the entry is marked medium.
What would resolve it
Decisions by European screening programmes on adopting tomosynthesis. No date is scheduled.

What the paper showed
Among 42 participants with progressing desmoid tumours randomised to three varegacestat regimens, the 1.2 mg once-daily dose produced a median tumour volume reduction of 51.91% at week 16 and a confirmed objective response rate of 35.7%, against 21.4% for each intermittent regimen. Among the 29 participants who entered the open-label extension on 1.2 mg daily, the median best tumour volume change was -85.88% and the confirmed response rate 57.1%; 21.4% of participants had grade 3 treatment-related adverse events and none had serious ones.
Why it scores where it does
This paper is the dose-finding basis for the phase 3 that met its primary endpoint in December 2025 and for the application the FDA accepted in July 2026 with an action date of 28 April 2027, which would make this the second approved gamma-secretase inhibitor in a rare tumour market held by one drug. The sponsor's value rests on this asset, and the chosen dose produced deep and durable shrinkage. The phase 2 results were first presented at ASCO in June 2023 and updated at ESMO in 2023 and 2024, so the paper is backward-looking, and the competing drug's owner is a large diversified company.
What would resolve it
FDA action date of 28 April 2027 for varegacestat in desmoid tumours. A European application is planned by the end of 2026.
Among 205 adults with overweight or obesity and without diabetes treated for 24 weeks, least-squares mean body weight change was -13.6% to -19.7% across once-weekly UBT251 doses of 2 mg to 6 mg against -2.0% with placebo, with placebo-adjusted differences of -11.5% (95% CI, -14.1 to -9.0) to -17.7% (95% CI, -20.8 to -14.6), all P<0.001. Waist circumference, blood pressure, lipids and glycaemic measures improved, and the most frequent adverse events were mild to moderate gastrointestinal symptoms.
Why it scores where it does
Weight loss near the top of the incretin class after only 24 weeks, now in print, supports the licensee's decision to take a triple agonist outside Greater China as a follow-on to its injectable franchise and keeps the Chinese owner's pipeline credible. The placebo-adjusted effect is large and dose-ordered in a double-blind trial. The topline was released on 24 February 2026, the trial is China-only and 24 weeks, the licensee is large enough that one phase 2 asset is a low exposure, and phase 3 plans rather than these data will set the value.
What would resolve it
Start of a Chinese phase 3 and the licensee's global phase 1b/2a trial, with topline guided to 2027. No date is publicly scheduled.

What the paper showed
Ten participants blind from advanced retinitis pigmentosa received a single intravitreal injection of an AAV vector encoding ChrimsonR in the worse-seeing eye and used light-stimulating goggles. There were 34 ocular adverse events in 9 participants, 23 mild, 10 moderate and one severe (a transient central retinal artery occlusion immediately after injection that resolved within minutes), and light sensitivity increased in 7 participants by factors of 2.0 to 62.3, with 6 reaching a threshold the authors define as clinically meaningful but did not prespecify.
Why it scores where it does
An NEJM publication of first-in-human optogenetic vision restoration gives a micro-cap's second asset top-tier validation in a disease with no approved therapy at late stages, and for a company of that size the asset is close to the whole story. Seven of ten participants gained light sensitivity and the one severe event resolved within minutes. The study is ten patients with safety as the primary outcome, the improvement threshold was taken from the literature rather than prespecified, the one-year results were first announced in February 2023, and the sponsor is a micro-cap whose continuing operations depend on raising capital, so its shares answer to financing as much as to data.
What would resolve it
No scheduled regulatory or readout event for the programme. The next marker would be a further trial or a partnership.

What the paper showed
Among 810 adults in China with type 2 diabetes inadequately controlled on metformin, treatment differences against dapagliflozin 10 mg in glycated haemoglobin change at week 32 were -0.30, -0.22 and -0.40 percentage points with safiglipron 30 mg, 60 mg and 90 mg, non-inferior at every dose (all P<0.0001; margin 0.4), and 90 mg met the superiority criterion (difference -0.25; 95% CI, -0.45 to -0.05; one-sided P=0.0067). Mean body weight changes were -2.35%, -3.55% and -4.17% against -3.60% with dapagliflozin, and adverse events led to discontinuation in about 4% against 1.5%.
Why it scores where it does
A second pivotal trial of the first Chinese oral small-molecule GLP-1 receptor agonist is in print a week after the first, with a Nasdaq-listed licensee holding rights outside Greater China and a Chinese marketing application the sponsor has said it intends to file. Non-inferiority on glycated haemoglobin was met at every dose and superiority at the top one in an 810-patient double-blind trial. The result was in the sponsor's and licensee's topline release of 7 July 2026 and was presented at the European diabetes congress in September, the comparator is an SGLT2 inhibitor rather than another incretin, weight loss beat it only at the top dose, and the licensee names an injectable dual agonist as its lead programme.
What would resolve it
The Chinese regulatory decision on safiglipron once an application is accepted, and the licensee's global phase 2 obesity data expected in 2027. No date is publicly scheduled.

What the paper showed
Among 1,870 Danish patients with relapsing-remitting multiple sclerosis starting ofatumumab (1,385) or ocrelizumab (485) between January 2022 and November 2025, the weighted intention-to-treat annualised relapse rate was 0.05 (95% CI, 0.04 to 0.06) against 0.03 (0.02 to 0.04), a rate ratio of 1.65 (1.09 to 2.50; P=0.018). Time to first relapse (hazard ratio 1.50; 95% CI, 0.99 to 2.29), disability worsening, progression independent of relapse and MRI lesion activity did not differ significantly.
Why it scores where it does
A nationwide head-to-head comparison of two anti-CD20 blockbusters, favouring the infused incumbent over the self-injected challenger on relapse rate, gives prescribers and payers a concrete data point in a share contest worth billions a year. The registry captures every treated patient in the country and the weighting adjusts for measured confounders. The design is observational with a risk of indication bias the authors acknowledge, follow-up is about two years, absolute relapse rates were very low in both groups, no secondary outcome differed, and the authors themselves call the difference of limited clinical relevance; both companies are mapped by product identity and both are diversified.

What the paper showed
Among 67 patients with HRD-positive stage III to IV high-grade serous or endometrioid ovarian cancer judged unlikely to achieve complete primary debulking, two cycles of neoadjuvant niraparib produced an objective response in 30 of 48 response-evaluable patients (63%; 95% CI, 47 to 76) and R0 resection in 32 of 40 operated patients (80%; 95% CI, 64 to 91). Grade 3 to 4 thrombocytopenia occurred in 48%, serious adverse events in 28%, and there were no treatment-related deaths.
Why it scores where it does
A chemotherapy-free neoadjuvant PARP strategy reaching an 80% complete resection rate would extend the use of a drug that anchors its Chinese licensee's commercial business and could move PARP inhibition ahead of surgery. Both co-primary endpoints were met at a median follow-up of 37.7 months, and the licensee is named in the funding statement. The trial is single-arm, the authors state that randomised validation against platinum-taxane chemotherapy is required and a randomised successor trial is registered, haematological toxicity was substantial, and response data from this cohort were published in Cell in July 2024 with the licensee's own release, so the primary analysis confirms rather than reveals.
