Six of the ten are incretin trials from the diabetes congress week, led by a dual agonist whose full results were new to everyone at once
The week
The European diabetes congress met this week and the journals published around it: survodutide in type 2 diabetes, two retatrutide phase 3 trials, the orforglipron cardiovascular safety trial, a Chinese oral GLP-1 and a semaglutide kidney mechanism study take six of the ten places. Most of those results were topline months ago, which is why a randomised oncolytic virus trial in pancreatic cancer and two oncology papers with December FDA dates rank above all but one of them. Seven of the ten results were public before their papers; the survodutide, safiglipron and semaglutide kidney results were not.

What the paper showed
Among 752 adults with type 2 diabetes and a body-mass index of 27 or more, mean weight change at week 76 by the treatment-regimen estimand was -8.2% with survodutide 3.6 mg, -9.8% with 6.0 mg and -3.9% with placebo, and weight reduction of at least 5% occurred in 57.6%, 64.5% and 35.1% (P<0.001 for all comparisons with placebo). Glycated haemoglobin fell 0.9 and 0.8 percentage points against 0.2 with placebo from a baseline of 7.4%, and gastrointestinal adverse events occurred in 72.8%, 77.7% and 38.6%.
Why it scores where it does
The second pivotal obesity trial of the only glucagon and GLP-1 dual agonist at this stage was disclosed in full for the first time, with the sponsor's and partner's releases, the congress presentation and the paper landing on the same day, so nobody had the dataset before anyone else. Both primary endpoints were met in a 752-patient double-blind phase 3, and the result completes the core SYNCHRONIZE package with the population that is hardest to treat. The partner's release leads with a 13.1% figure from the efficacy estimand; the treatment-regimen figures in the abstract are the ones above, and they sit well below what the leading incretins have reported in people with type 2 diabetes. Three quarters of treated patients had gastrointestinal events, and the sponsor is private, so listed exposure runs through the Danish partner that co-developed the molecule and whose value rests on it and on one other asset.
What would resolve it
Results of the SYNCHRONIZE-CVOT cardiovascular outcomes trial, which the sponsor says it expects to present later in 2026, and any regulatory submission announcement. No date is publicly scheduled.

What the paper showed
In first-line metastatic pancreatic ductal adenocarcinoma, median overall survival was 10.6 against 8.6 months in the intention-to-treat population (hazard ratio 0.69; 95% CI, 0.42 to 1.12; P=0.196) and 10.8 against 8.6 months in the full analysis set of 48 per group (hazard ratio 0.57; 95% CI, 0.34 to 0.96; P=0.055), which the authors state met the primary endpoint. Progression-free survival in that set was 7.0 against 4.6 months (hazard ratio 0.55; P=0.011), 18-month survival was 31.1% against 8.5%, and serious VCN-01-related adverse events occurred in 22.6% of patients.
Why it scores where it does
A randomised survival signal in first-line metastatic pancreatic cancer is rare, and for a company whose value is concentrated in this one oncolytic virus, a full peer-reviewed dataset in a leading journal is the credential it needs before a phase 3 or a partner. The hazard ratios, a doubling of duration of response from 5.4 to 11.2 months and the tail at 15 and 18 months support the claim. The intention-to-treat comparison did not reach significance, the full-analysis-set result is declared to have met the endpoint by the authors at a P value of 0.055, the trial is about 100 patients, and the headline figures were released by the company on 7 May 2025, so the new material is the intention-to-treat analysis and the survival rates. The company said in March 2026 that the FDA agreed with its phase 3 design and that it is seeking funding or a partner to run it.
What would resolve it
A funded or partnered phase 3 trial following the March 2026 end-of-phase-2 agreement with the FDA. No date is publicly scheduled.

What the paper showed
Among 373 randomised patients whose ER-positive, HER2-negative advanced breast cancer had progressed after a CDK4/6 inhibitor plus endocrine therapy, median progression-free survival was 10.0 months with giredestrant plus everolimus against 5.5 months with standard endocrine therapy plus everolimus in the 207 patients with ESR1-mutated tumours (hazard ratio 0.38; 95% CI, 0.27 to 0.54; P<0.001), and 8.8 against 5.5 months overall (hazard ratio 0.56; 95% CI, 0.44 to 0.71; P<0.001). Adverse events occurred in 98.9% and 96.8% of patients, most commonly stomatitis in roughly half of each group.
Why it scores where it does
The pivotal paper for an all-oral regimen after CDK4/6 failure arrives with a United States decision inside the quarter: the FDA accepted the application in February 2026 with an action date of 18 December 2026, and a second giredestrant application in the adjuvant setting carries a 30 November date. Progression-free survival nearly doubled in the ESR1-mutated population in a randomised phase 3 of 373 patients. The trial is open-label, the endpoint is investigator-assessed progression rather than survival, the data have been public since the sponsor's topline release and the ESMO presentation in autumn 2025, and the sponsor is large enough that one breast cancer asset is not a top value driver. The competitors listed market or develop rival oral SERDs and are mapped by drug class rather than by anything in the paper.
What would resolve it
FDA action date of 18 December 2026 for giredestrant plus everolimus in ESR1-mutated advanced breast cancer, and 30 November 2026 for adjuvant giredestrant.

Among 48 patients in the United States with relapsed or refractory primary central nervous system lymphoma after at least one high-dose methotrexate regimen, the independently assessed overall response rate was 67% (95% CI, 52 to 80) and the complete response rate 44% (95% CI, 29 to 59), with a median duration of response of 9.3 months and a median time to response of 1.0 month. Grade 3 or higher treatment-related adverse events occurred in 27% of patients, and one death from bronchopulmonary aspergillosis was treatment related.
Why it scores where it does
This is the dataset behind a United States application under FDA review for a disease with no approved therapy; the sponsor's subsidiary announced acceptance in February 2026 with an action date of 18 December 2026 under accelerated approval. Two thirds of patients responded and nearly half completely, with responses arriving within a month and lasting a median of nine months. The trial is single-arm with 48 patients and a median follow-up under a year, the headline response rates were presented at ASCO in 2025, and the drug is already marketed in Japan by a parent for which a rare United States indication is a small line, so the live element is the decision rather than the data.
What would resolve it
FDA action date of 18 December 2026 for tirabrutinib in relapsed or refractory primary CNS lymphoma.

What the paper showed
Among 284 adults in China with type 2 diabetes managed by diet and exercise alone (mean baseline glycated haemoglobin 7.95%), placebo-adjusted reductions in glycated haemoglobin at week 32 were 1.22, 1.20 and 1.45 percentage points with once-daily safiglipron 30 mg, 60 mg and 90 mg (all P<0.0001), and 71.4% to 77.8% of treated patients reached a level below 7.0% against 25.0% on placebo. Placebo-adjusted weight change was -0.65%, -2.23% and -3.56%, and adverse events led to discontinuation in 1.4%, 2.9% and 6.9% of patients on the three doses and none on placebo.
Why it scores where it does
This is the first full phase 3 publication for an oral small-molecule GLP-1 receptor agonist developed in China, and it is a first disclosure: the July 2026 topline release from the sponsor and its licensee covered the sibling obesity and metformin add-on trials, not this one, which was presented at the European diabetes congress the same week the paper appeared. The glycaemic effect is clean and dose-ordered in a lean, early-diabetes population, six months after the first oral non-peptide GLP-1 reached the United States market. The trial is single-country with about 70 patients per arm, weight loss is modest, and neither listed company depends on this molecule: the sponsor is a diversified Chinese pharmaceutical group, and its Nasdaq-listed licensee, which holds rights outside Greater China and completed its initial public offering in April 2026, names an injectable dual agonist as its lead programme.
What would resolve it
A Chinese new drug application for safiglipron in type 2 diabetes, which the sponsor has said it intends to file, and data from the licensee's global phase 2 obesity trial, expected in 2027. No date is publicly scheduled.

Among 275 treatment-naive patients with chronic hepatitis C of any genotype, 13% with compensated cirrhosis and 27% with baseline NS5A resistance substitutions, 8 weeks of bemnifosbuvir 550 mg plus ruzasvir 180 mg produced a sustained virological response at 12 weeks in 90% (248 of 275) of the intention-to-treat population and 98% (210 of 215) of the adherent per-protocol population. Five per-protocol patients relapsed, and no drug-related serious adverse events or discontinuations for drug-related adverse events were reported.
Why it scores where it does
The peer-reviewed phase 2 record arrives as a single-asset company approaches its second phase 3 readout: the North American trial met non-inferiority to sofosbuvir-velpatasvir in July 2026 and the trial outside North America is guided to around year-end, so nearly all of the company's value rests on this regimen. Cure rates were high regardless of genotype or baseline resistance in an eight-week course. The intention-to-treat rate of 90% reflects adherence attrition rather than virological failure, these results were first released by the company in December 2024, and the incumbent regimens from two large competitors leave little room for a late entrant beyond duration and price.
What would resolve it
Topline results of the C-FORWARD phase 3 trial outside North America, guided by the company to around year-end 2026. No exact date is published.

What the paper showed
Among 2,339 adults with obesity and without diabetes, mean weight change at 80 weeks was -17.6%, -23.7% and -25.0% with retatrutide 4 mg, 9 mg and 12 mg against -3.9% with placebo (differences for the two higher doses -19.8 and -21.0 percentage points; P<0.001 for both). In the 574 participants with knee osteoarthritis, pain scores fell 3.5 and 3.6 points on the two higher doses against 1.9 with placebo under the hybrid estimand, and in the 243 with obstructive sleep apnoea the apnoea-hypopnoea index fell 34.3 and 31.7 events per hour against 9.9 (all P<0.001).
Why it scores where it does
The full TRIUMPH-1 paper adds the knee osteoarthritis and sleep apnoea co-primary results to weight numbers that have been public since the sponsor's topline release on 21 May 2026, and those two endpoints are what would separate a triple agonist label from the incretins already on the market. The weight effect sits at the top of the reported phase 3 range for the class, in a 2,339-patient double-blind trial. The sponsor has guided its regulatory submission to the first quarter of 2027, outside any near window, the headline was among the most covered obesity results of the year and has been priced for months, and the sponsor is diversified enough that one more incretin is a medium rather than a high exposure.
What would resolve it

Effects of semaglutide on kidney disease in type 2 diabetes: a randomized placebo-controlled trial

What the paper showed
Among 106 participants with type 2 diabetes and chronic kidney disease randomised to 52 weeks of semaglutide 1 mg weekly or placebo, the coprimary magnetic resonance outcomes of kidney oxygenation, global perfusion and tissue inflammation were not significantly altered by treatment. Secondary outcomes showed a reduced renal artery resistive index and a stabilised apparent diffusion coefficient with semaglutide, and single-nucleus and spatial transcriptomics on paired biopsies showed effects concentrated in glomerular endothelial cells with fewer immune cells nearby.
Why it scores where it does
Tissue-level evidence for how semaglutide protects the kidney underpins a renal label claim that helps distinguish the sponsor's principal asset as oral and injectable competition multiplies, and paired before-and-after kidney biopsies with transcriptomics are rare in a randomised drug trial. The vascular and molecular secondary findings point in one direction. The coprimary imaging endpoints were not met, the trial has 106 participants with biopsies in 33, and the commercial case for kidney protection already rests on the large outcomes trial reported in 2024, so this paper explains rather than changes anything. A summary that called it a positive trial would overstate it.
What would resolve it
No resolving event. Label and guideline language on kidney protection evolve slowly, and nothing is scheduled.

What the paper showed
Among 1,152 adults with obesity and type 2 diabetes, mean weight change at week 80 by the treatment-regimen estimand was -11.9%, -16.8% and -18.8% with retatrutide 4 mg, 9 mg and 12 mg against -5.1% with placebo (differences -6.9, -11.8 and -13.8 percentage points; P<0.0001 for all). Permanent discontinuation for adverse events or death was 12% at 9 mg and 8% at 12 mg against 5% with placebo, with hypotension in up to 6% and dysaesthesia in up to 7% of treated participants against 1% or less on placebo.
Why it scores where it does
TRIUMPH-2 carries the triple agonist's weight effect into the diabetes population, where every incretin loses efficacy, at magnitudes above what dual agonists have reported there, in a 1,152-patient double-blind trial in which 84% completed study drug. The dose-ordered placebo-adjusted differences are the support. Topline was released on 23 July 2026, the filing is guided to the first quarter of 2027, and the tolerability detail now in print, with a discontinuation rate at 9 mg more than double that of placebo and hypotension and dysaesthesia appearing on drug, is the part competitors will quote.
What would resolve it

What the paper showed
Among 2,749 adults with type 2 diabetes and established cardiovascular or chronic kidney disease followed for a median of 2 years, four-component major adverse cardiovascular events occurred in 57 of 1,358 (4.2%) on orforglipron and 67 of 1,343 (5.0%) on insulin glargine (hazard ratio 0.84; 95% CI, 0.59 to 1.20; P<0.0001 for non-inferiority). Clinically significant or severe hypoglycaemia occurred in 6.8% against 19.2%, gastrointestinal adverse events in 62.1% against 14.2%, and 19 (1.4%) against 43 (3.2%) participants died.
Why it scores where it does
Cardiovascular safety is the gating evidence for a type 2 diabetes label on the first oral non-peptide GLP-1 receptor agonist, which the sponsor launched for obesity in April 2026 and submitted for diabetes in the second quarter, so this paper is the record behind a pending indication. The event rate was numerically lower than with insulin, with a third of the hypoglycaemia and fewer deaths, in a 2,749-patient event-driven trial. The design is open-label against an active comparator and powered for non-inferiority with a margin of 1.8, the result has been public since the sponsor's topline release in April 2026, and gastrointestinal events in 62% of treated patients were the main reason for discontinuation.
