The strongest trials this week made the case for doing less, from fewer colonoscopies to two fewer cycles of chemotherapy

The week

A B7-H3 antibody-drug conjugate cut the risk of death by more than half in relapsed small-cell lung cancer and leads the list, but the pattern of the week is de-escalation: surveillance colonoscopy at five years rather than three, blinatumomab in place of two chemotherapy cycles in high-risk childhood leukaemia, and no cognitive case for revascularising asymptomatic carotid stenosis. Several of the strongest results were public before their papers: the leukaemia trial at a haematology congress in June, the adolescent HIV trial at a conference in July, the head and neck immunotherapy survival data in an investor release last August.

What the paper showed

Among 451 patients with small-cell lung cancer progressing after first-line platinum therapy, median overall survival was 13.3 months with tambotatug pelitecan against 9.4 months with topotecan (stratified hazard ratio for death 0.46; 95% CI, 0.35 to 0.62; P<0.001). Confirmed objective response occurred in 59.1% against 9.7%, and grade 3 or higher adverse events in 55.4% against 77.9%.

Why it scores where it does

B7-H3 is now a validated target in relapsed small-cell lung cancer, and an antibody-drug conjugate against it beat the long-standing comparator by a wide margin. This is a randomised phase 3 interim analysis with a large survival separation, a response rate six times that of topotecan and less severe toxicity than the control arm, published as a first disclosure with the collaborator's own release following a day later. The trial enrolled a Chinese population against topotecan rather than against more recently approved options, so generalisability and regulatory acceptance elsewhere are open, the developer is private, and the listed collaborator is a large diversified company for which one asset is not a top-three value driver. A second B7-H3 conjugate reported a positive phase 3 in the same indication at nearly the same time; that result was not verified here and no comparison is drawn.

What the paper showed

Among 53 checkpoint inhibitor naive patients, the objective response rate by masked central review was 34.0%, median progression-free survival 5.3 months (95% CI, 2.10 to 9.00) and median overall survival 39.3 months (95% CI, 23.90 to not evaluable). Among 22 checkpoint inhibitor resistant patients there were no objective responses and the primary endpoint was not met, with median overall survival of 14.8 months (95% CI, 8.50 to 26.00).

Why it scores where it does

This is the lead asset of a small company, so the paper bears on nearly all of its value, and the checkpoint-naive survival figure is well beyond what checkpoint blockade alone has historically produced in this disease. Central review put the response rate in the mid-thirties with grade 3 or higher treatment-related events in 13.8%. The limits are substantial: no randomised comparator, a checkpoint-resistant cohort that formally failed its primary endpoint, a small and 95% male population, and cross-trial survival comparisons in head and neck cancer are unreliable. The company released the final topline survival data on 25 August 2025, so the publication adds peer review and the failed cohort in full rather than new direction.

What would resolve it

Colonoscopy Intervals and Colorectal Cancer Incidence after Adenoma Removal

Market Sensitivity Score 56/100

What the paper showed

Among 10,799 randomised patients with high-risk adenomas, the five-year cumulative incidence of colorectal cancer was 0.77% when surveillance colonoscopy began at five years against 0.82% when it began at three, a difference of -0.05 percentage points with an upper confidence bound of 0.68 that met the prespecified noninferiority criterion. Five patients died of colorectal cancer, 3 (0.06%) in the five-year group and 2 (0.04%) in the three-year group.

Why it scores where it does

Surveillance colonoscopy after high-risk adenoma removal can begin at five years rather than the three that guidelines recommend, with no detectable rise in cancer incidence, which over time means fewer procedures per patient. The trial is large, multinational and randomised, and met its margin with tight bounds. This is a 5.5-year interim on a ten-year primary endpoint, inverse probability weighting was used to handle non-participation, and guideline bodies move slowly, so any change in procedure volume would be gradual. No company appears in the author list or funding, which names a national research council; the listed names are the leading endoscope maker and a provider of non-invasive alternatives, mapped by category rather than involvement.

What would resolve it

Guideline committee responses and the final ten-year analysis. No date is publicly scheduled for either.

Oral KRAS G12D inhibitor GFH375 for previously treated advanced solid tumors with KRAS mutations: a phase 1 trial

Market Sensitivity Score 55/100

  • Attention10

What the paper showed

Among 74 heavily pretreated patients with KRAS-mutant advanced solid tumours, the confirmed objective response rate was 35.1% (13 of 37) in pancreatic ductal adenocarcinoma, with median progression-free survival of 5.5 months and median overall survival of 9.9 months, and 35.7% (5 of 14) in non-small-cell lung cancer. Grade 3 or higher treatment-related adverse events occurred in 36.5% (27 of 74), including one grade 5 event reported as septic shock.

Why it scores where it does

An oral inhibitor that engages both the active and inactive states of KRAS G12D produced reproducible single-agent responses in pancreatic cancer, where that mutation dominates and options are few, and the listed partner has made this molecule the lead programme in its collaboration with the private developer. Response rates in the mid-thirties in a heavily pretreated population, with no maximum tolerated dose reached, support the claim. This is phase 1, single-arm and conducted in China, the confirmed response denominators are small, and earlier cuts of the same dataset were disclosed through investor releases and conference presentations during 2025, so the publication confirms rather than reveals.

What would resolve it

Phase 2 expansion cohort readouts in pancreatic, lung and colorectal cancer, and progress of the separate United States trial of the same molecule. No readout date is publicly scheduled.

Blinatumomab for Replacing Chemotherapy in Pediatric Acute Lymphoblastic Leukemia

What the paper showed

Among 709 randomised children with newly diagnosed high-risk B-cell acute lymphoblastic leukaemia, estimated four-year event-free survival was 83.0% (95% CI, 77.4 to 87.4) with two cycles of blinatumomab replacing two cycles of chemotherapy against 70.3% (95% CI, 63.8 to 75.9) with chemotherapy alone (P=0.0002), a hazard ratio for an event of 0.51 (95% CI, 0.35 to 0.73). Treatment-related infection occurred in 23.9% against 69.4%, and neurotoxic events in 12.0% against 3.2%.

Why it scores where it does

A T-cell engager displaced two cycles of the most toxic conventional chemotherapy in frontline high-risk childhood leukaemia rather than serving as salvage, with a large event-free survival gap and a third of the infection burden. The trial was academic, run by a European cooperative group and funded by charities; the company enters because it owns and markets the drug, not as sponsor. This is a planned interim at a median 2.9 years of follow-up, neurotoxicity was higher with blinatumomab, and frontline paediatric leukaemia is a small population against the company's revenue base, so the question is label breadth and duration of use rather than a step change in value. The results were presented in plenary at the European Hematology Association congress in June 2026, so the paper is the record.

What would resolve it

Whether a regulatory filing for a frontline paediatric indication follows, and the final analysis at longer follow-up. No regulatory date is publicly scheduled.

Revascularization of Asymptomatic Carotid Artery Stenosis: Cognitive Results of the CREST-2 Randomized Trials

Market Sensitivity Score 53/100

  • Exposure5

What the paper showed

Among 2,165 participants completing cognitive assessment, the fully adjusted composite z score at four years did not differ by treatment in either trial: 0.22 (95% CI, 0.13 to 0.30) for stenting plus intensive medical management against 0.18 (95% CI, 0.09 to 0.27) for medical management alone, a difference of 0.03 (95% CI, -0.09 to 0.15). For endarterectomy the difference was 0.02 (95% CI, -0.09 to 0.14), and no individual cognitive test differed significantly.

Why it scores where it does

Cognitive benefit is not a valid reason to revascularise asymptomatic carotid stenosis. That comes from prespecified secondary analysis of two large randomised trials with blinded central cognitive assessment, a prespecified minimal clinically important difference and null results on every individual test. It addresses one rationale only; stroke prevention remains the primary indication and was not the subject of this analysis, so any effect on procedure volume is confined to borderline cases, and an accompanying commentary argued revascularisation may still be justified on other grounds. The paper names no manufacturer; the listed companies are the established suppliers of carotid intervention hardware, mapped by category.

What would resolve it

Whether professional society guidance on asymptomatic carotid intervention is revised. No date is publicly scheduled.

Factor IX Padua AAV gene therapy in adolescents with hemophilia B: a phase 1 trial

Market Sensitivity Score 49/100

  • Novelty11
  • Attention14

What the paper showed

Eleven adolescents aged 12 to 18 with severe or moderately severe haemophilia B received a single dose and were followed for 52 weeks with no dose-limiting toxicity. Mean factor IX activity at week 52 was 41.8 IU per decilitre (standard deviation 30.1), and the mean annualised bleeding rate fell from 13.9 to 0.5.

Why it scores where it does

A factor IX Padua gene therapy already approved for adults in China appears safe and active in adolescents, extending the addressable population to patients with the most remaining years of prophylaxis to displace. All eleven participants completed 52 weeks with clinically meaningful factor activity and a large fall in bleeding. It is a single-arm phase 1 of eleven patients with no control group, one participant needed immunosuppression for abnormal liver function, the study was conducted in China, and the developer is private. The listed partner holds the exclusive commercial licence for mainland China, Hong Kong and Macau, which is a documented link but a small one for a company of that size; the competitors market approved gene therapies for the same disease.

What would resolve it

Regulatory progress for adolescent use in China and any partner commentary on other territories. No date is publicly scheduled.

A single-cell atlas of large B cell lymphomas reveals distinct malignant archetypes predictive of clinical outcomes

Market Sensitivity Score 47/100

  • Evidence12
  • Exposure4
  • Catalyst4
  • Novelty12
  • Attention15

What the paper showed

Single-cell RNA, B-cell receptor and T-cell receptor sequencing of 63 large B-cell lymphoma biopsies identified five conserved transcriptional archetypes of malignant cells that co-occur within individual tumours. A high abundance of one archetype, marked by memory B-cell features and quiescence, correlated with poor event-free survival after standard immunochemotherapy, and the association held in independent cohorts by deconvolution of bulk sequencing data.

Why it scores where it does

Which malignant cell states a tumour contains, rather than its tumour-level classification alone, predicts who fails standard immunochemotherapy in a disease where up to 40% of patients relapse or prove refractory within two years. The atlas is multimodal and the survival association was validated in independent cohorts. Sixty-three cases is a modest discovery set, the finding is associative and not yet a decision tool, and no product or trial is attached to it. Several authors list the company as their affiliation, which is direct involvement in the work rather than an inference from product overlap, but it does not by itself identify an asset.

What would resolve it

Whether the stratification is taken into a prospective trial. Nothing is scheduled.

Beyond Traditional Glycemic Measures: CGM Glycemic Profiles Reveal Associations With Cardiometabolic Risk in Individuals Without Diabetes

Diabetes Care · cross-sectional cohort analysis · · devices · confidence high

Market Sensitivity Score 45/100

  • Evidence12
  • Exposure6
  • Catalyst4
  • Novelty10
  • Attention13

What the paper showed

Among 1,356 Framingham Heart Study participants without diabetes or cardiovascular disease who wore a blinded Dexcom G6 Pro monitor for up to ten days, one standard deviation more time above 140 mg/dL (15.4%) was associated with 2% higher ten-year cardiovascular risk estimates and 21% to 26% higher odds of hypertension and dyslipidaemia after adjusting for fasting glucose. Profiles of higher monitor-defined dysglycaemia carried 6% to 7% higher ten-year risk estimates and captured 48.1% to 84.2% of people with conventionally defined prediabetes.

Why it scores where it does

Continuous glucose monitoring carries cardiometabolic risk information in people who do not have diabetes, which is exactly the population the device makers need to adopt the technology for the non-diabetic market to matter. The cohort is large and well characterised and the associations survived adjustment for fasting glucose. The design is cross-sectional, so it cannot show that monitoring changes any outcome, the effect sizes are modest, and the population is 92.6% non-Hispanic White. The abstract names the monitor used, which makes its maker a documented supplier to the study rather than a category guess; the second company is included as the principal competitor in the same market.

What would resolve it

Prospective outcome studies of monitoring in people without diabetes. Nothing is scheduled.

Switch to injectable cabotegravir-rilpivirine given every 8 weeks in adolescents living with HIV with virological suppression in sub-Saharan Africa (LATA): a randomised, open-label, multicentre, 96-week non-inferiority trial

What the paper showed

Among 476 randomised adolescents with virological suppression, 2 in the injectable group had a confirmed viral load of 50 copies per millilitre or higher by week 96, a Kaplan-Meier estimate of 0.9% (95% CI, 0.0 to 2.2), against 15 (6.4%; 95% CI, 3.7 to 9.8) on daily oral therapy, an estimated difference of -5.5 percentage points (99% CI, -10.3 to -1.5). The injectable regimen met noninferiority and then showed superiority (P=0.0013).

Why it scores where it does

Every-eight-week injectable therapy was not merely as good as daily pills in adolescents but better, in the group with the best-documented adherence difficulty and in the region holding most of the world's patients. The trial is randomised with 96-week primary follow-up, a median of 120 weeks observed, a superiority result and no new safety signals. The results were presented at the International AIDS Conference on 30 July 2026 with company releases the same day, so the paper confirms, and the commercial question in sub-Saharan Africa is access pricing rather than approval. The sponsor's majority-owned HIV unit developed cabotegravir; the second company's unit makes rilpivirine and is named in the funding statement.

What would resolve it

Access and pricing arrangements for the region. Nothing is publicly scheduled.
